While we provide highly skilled health care services to people in their homes and at Hospice House, an important part of our Mission is community wellness.

We offer a comprehensive array of programs on topics such as aging, healthy lifestyles, advance care planning, grief and bereavement, caregiving, and more throughout the communities we serve.

How we can help

Our experienced team partners with community members to help them meet their goals. There is no fee to attend the programs listed below.

  • People are living longer, and are in overall better health in their retirement years, however, societal expectations about aging and retirement years have made little progress. The purpose of this program is to change expectations and help participants live their best lives. The Aging Mastery Program offers older adults a fun and engaging way to gain skills and tools to manage their health, remain economically secure, and contribute actively in society as they age.

    Learn from local experts and each other about topics such as sleep, healthy eating, finances, and falls prevention. This 10-week nationwide program from the National Council on Aging is for older adults who want to increase:

    • Social connectedness
    • Physical activity levels
    • Healthy eating habits
    • Use of advance care planning

    This program is generously supported by the Bank of New Hampshire.

  • Falls are common as people age, but it does not have to be that way. Learn simple ways to reduce your risk of falling during this program. “A Matter of Balance” helps older adults learn to reduce their fear or falling and gain self-confidence to help them remain active. This eight-week nationwide program from Boston University is for older adults who want to:

    • View falls and fear of falling as controllable
    • Set realistic goals for increasing activity
    • Change their environment to reduce fall risk factors
    • Promote exercise to increase strength and balance
  • Healthy Families America-NH is a home visiting service available starting when you find out you are pregnant through your child’s 3rd birthday. It is non-judgmental support–when you want it–where you want. Healthy Families America-NH is free, convenient, voluntary and flexible.

  • Managing and/or caring for someone with a chronic condition such as arthritis, diabetes, hypertension, or any ongoing health condition can be challenging. Live Well, Be Well provides tools and information to help manage condition symptoms, maintain proper nutrition and exercise, communicate with your healthcare provider, and more. This six-week nationwide program developed by Stanford University as a collaborative research project by Stanford and the Northern California Kaiser Permanente Medical Care Program is for adults who want to be more prepared to:

    • Manage difficult emotions such as sadness, anger, or frustration
    • Deal with pain and extreme tiredness
    • Eat for better health
    • Talk with family, friends, and health professionals
  • Are you a family caregiver? “Powerful Tools for Caregivers” offers valuable tips to take care of yourself while tending to your loved one. This six-week program helps family caregivers learn to take care of themselves by:

    • Reducing personal stress
    • Changing negative self-talk
    • Communicating more effectively
    • Handling different feelings
    • Making challenging decisions

  • The Memory Cafe is a joyful and creative social experience for people with memory loss and their caregivers to gather with others in a positive and engaging environment. Cafes are offered in the Concord region.

    This program is generously supported by Northeast Delta Dental.

  • Caregiver Connection is an opportunity for family caregivers to support one another and connect with our team professionals. We look forward to providing a safe space to connect with other caregivers facing similar challenges.

  • Have your questions answered about caregiver resources, advance directives, durable power of attorneys for healthcare and/or living will, and in-home support.
