Concord Regional VNA Telehealth is advanced home monitoring technology that plays a key role in improving care for people who have a chronic condition (e.g. heart failure, diabetes, pulmonary condition, and hypertension) and who are at high-risk of re-hospitalization. Telehealth monitoring is used to augment nursing visits and engage you in necessary self-care to help you manage and prevent an exacerbation of your chronic condition.

Our skilled nurse at Concord Regional VNA reviews the information (e.g. blood pressure, weight, and blood sugar) that you electronically submit via the small, lightweight monitoring device in your home. The nurse then identifies medical changes and works with your physician on needed medication and treatment interventions.
