About Telehealth:

Telehealth is an advanced, home monitoring system that enables specially trained VNA nurses to assist chronically ill, frail, older adults in managing their illness from the convenience of their own home.

Our Telehealth monitoring system, known as the "Health Buddy", monitors patients with chronic illnesses such as heart failure, diabetes, pulmonary conditions, hypertension and who are at high risk for hospitalization. The telehealth monitoring is used to augment our nursing home visits and engage the patient in necessary self-care to help them to prevent an exacerbation of their illness.

Our "Health Buddy" Units and Telehealth Services include:

• A monitoring unit that can monitor blood pressure, weight, blood sugar, and blood oxygen levels.
• A telehealth nurse who is available seven days a week to assess the health and results of the patient's telehealth visits, communicate with the patient, their physician and their primary VNA nurse.
• Daily telehealth sessions provide the patient with education modules to reinforce teaching on their specific disease process and self-management techniques.
• Periodic clinical reports to the patient's physician on the data collected, enabling the physician to have clinical data to make quicker adjustment to medications and/or treatment when necessary.

The "Health Buddy" is designed to help our patients maintain their independence, reduce the risk of returning to the hospital and improve their health.

Who pays for Telehealth?

Telehealth Services are available for our Medicare Home Care and Hospice patients at no charge. Upon request from a patient or their physician a request for authorization will be made to the patient's third-party health insurance plan. If a patient or family would like to have telehealth monitoring and nursing oversight a monthly fee is available.
