Home Care is available for people of all ages in your own home, independent living community, assisted living community, and group home.

Managing Chronic Conditions

Our experienced team makes recommendations and gives you the tools to succeed in self-managing your chronic condition (e.g. congestive heart failure, diabetes, and chronic obstructive pulmonary disease).

Our Services Include:

  • Medication compliance
  • Physical and environmental safety assessments
  • Education and support
  • Nutrition counseling
  • Exercise plan
  • Information on needed lifestyle changes

If eligible, we also help you self-manage your chronic condition with telehealth monitoring. This small, lightweight device allows you to electronically submit your vital signs to our skilled nurse who reviews your information. The nurse identifies medical changes and works with your healthcare providers on needed medication and treatment interventions.

Administering Medication

Patients admitted to the hospital typically receive new medications or have changes made to their existing medications or both.

  • Help with organization and management of medications
  • Assistance with monitoring the effectiveness of medications and potential side effects
  • Collaboration with your physician and pharmacist to prevent dangerous medication interactions

Wound Care

Getting proper care for wounds that do not easily heal is critical in preventing infections and other serious complications.

  • Cleaning and changing wound dressings
  • Training patients and caregivers on how to change wound dressings, monitor healing progress, and detect potential complications

Injections

Our team serves and supports patients who need injections such as monthly Vitamin B12 injections.

  • Pre-measure medications to prevent dosage errors
  • Help on managing your medications
  • Collaboration with your physician to assess treatment and dosage

IV Therapy

Our team works closely with you and your caregiver to help you understand your disease or condition and become as independent as possible.

Monitoring Vital Signs

Monitoring basic vital signs allows our team to identify problems early and prevent them from becoming serious.

Checking your vital signs such as your blood pressure, pulse, body temperature, respiration rate, and weight

  • Teaching you how to use equipment such as a blood pressure cuff
  • Providing self-care and educational materials about your specific illness
  • Helping you evaluate changes and recognize potential complications

Physical Therapy

Our physical therapists help you regain your mobility that gives you greater independence.

Our Services Include:

  • Assistance with moving in and out of bed or a chair
  • Instructions on using a cane or walker safely and effectively
  • Education to improve your posture and balance
  • Ways to conserve energy and limit strain on joints and muscles
  • Information about falls prevention and exercise programs

Occupational Therapy

Our occupational therapists develop a specialized plan to meet your needs and work with you to relearn the motor skills you need to meet your goals.

Our Services Include:

  • Improvement of your motor skills such as buttoning your shirt, putting on shoes, or cutting food becomes easier
  • Strategies or specially designed equipment to help you complete basic tasks and activities of daily living safely
  • Exercise and positioning programs to help you recover from surgery

Speech Therapy

Speech therapists help develop a program to strengthen the muscles of your mouth, neck, and face to alleviate problems such as choking and coughing, while improving the ability to speak clearly, chew, and swallow.

Our Services Include:

  • Help so you understand and process what is being said
  • Learn other ways to communicate after a stroke
  • Refine reading, writing, performing calculations, and remembering things
  • Instructions on how to cope when communication is difficult or problematic

Our goals are to increase your functional levels and improve your quality of life to help keep you at home and not back in the hospital.
