Concord Regional VNA Home Care is for people who are recovering from surgery or an accident, recovering from exacerbation of a chronic illness, coping with physical impairments, dealing with the complications of aging, prescribed a new medication, and diagnosed with a new condition.
Home Care is available for people of all ages in your own home, independent living community, assisted living community, and group home.
Our experienced team makes recommendations and gives you the tools to succeed in self-managing your chronic condition (e.g. congestive heart failure, diabetes, and chronic obstructive pulmonary disease).
If eligible, we also help you self-manage your chronic condition with telehealth monitoring. This small, lightweight device allows you to electronically submit your vital signs to our skilled nurse who reviews your information. The nurse identifies medical changes and works with your healthcare providers on needed medication and treatment interventions.
Patients admitted to the hospital typically receive new medications or have changes made to their existing medications or both.
Getting proper care for wounds that do not easily heal is critical in preventing infections and other serious complications.
Our team serves and supports patients who need injections such as monthly Vitamin B12 injections.
Our team works closely with you and your caregiver to help you understand your disease or condition and become as independent as possible.

Our physical therapists help you regain your mobility that gives you greater independence.
Our occupational therapists develop a specialized plan to meet your needs and work with you to relearn the motor skills you need to meet your goals.
Speech therapists help develop a program to strengthen the muscles of your mouth, neck, and face to alleviate problems such as choking and coughing, while improving the ability to speak clearly, chew, and swallow.
Our goals are to increase your functional levels and improve your quality of life to help keep you at home and not back in the hospital.
