Medical Social Worker

Professional - Part Time - Benefitted

The Concord Regional VNA is seeking a part-time (24 hours per week) MSW to provide counseling and support of Hospice patients and families. Responsibilities include acting as a resource to outside agencies and referring patient, caregiver and/or family members to appropriate community resources and financial assistance as needed. Assist team members in understanding the significant social, emotional and ethical factors related to a patient's/family's health concern and dying process. Provide short-term problem-oriented therapeutic and/or supportive counseling and casework services to patients and families as needed from the period of admission to one year after death (bereavement). Work with a great team of health care professionals!

Qualified candidates must have a Master’s in Social Work.

Work Schedule: Generally Monday - Friday with flexibility to work occasional evenings and weekends to meet program needs.




 

RN - Home Care

Professional - Full Time - Part Time - Benefitted

The Concord Regional Visiting Nurse Association is seeking a Registered Nurse to deliver skilled nursing care to clients and families according to nursing standards. Responsibilities include implementing a plan of care and managing a caseload of patients. Travel, weekend and holiday rotation is required for this position. The qualified candidate will have a RN diploma and Associate Degree (BSN preferred); current NH nursing license; recent acute care experience skills and a minimum of two years nursing experience.

Must be able to understand and carry out directions and instructions, record messages and keep records. Is able to read, write and speak English. Is physically and emotionally capable of performing the job requirements.

Work Schedule includes rotating weekends, holiday and backup on-call responsibilities.
- PT (32 hrs/wk; Tuesday – Friday 11 am - 7 pm)
- FT (37.50 hrs/wk; Monday – Friday 8 am – 4 pm)

 

RN - Pediatric

Responsibilities include implementing a plan of care. Travel is required for this position. Licensed Nursing Assistant - Hospice

Paraprofessional - Full Time - Part Time - Per Diem - Benefitted - Non-Benefitted

A LNA performs personal care and designated health-related services for Hospice patients.

The care is provided to patients under the supervision of a registered nurse. MINIUMUM OF 1 YEAR OF LNA EXPERIENCE and NH L.N.A. LICENSE IS REQUIRED.

Is able to understand and carry out directions and instructions, record messages and keep records. Is able to read, write and speak English. Is physically and emotionally capable of performing the job requirements.

Positions are available for both the Hospice House and community. (Travel is required for all community positions.)

Full Time (11 pm - 7 am)

RN- Hospice House

A skilled RN to care for patients at our Hospice House with end of life care needs. The qualified candidate will be licensed by the State of New Hampshire as a RN with a MINIMUM OF 2-YEARS RECENT ACUTE CARE EXPERIENCE SKILLS IS REQUIRED. Previous Hospice and/or oncology experience preferred. Position includes weekend, holiday, on-call and off-shift rotations as needed by the program.

Personal Care Service Provider

Paraprofessional - Per Diem - Non-Benefitted

We are seeking a Personal Care Service Provider to provide assistance with eating, bathing, dressing, personal hygiene, activities of daily living, preparation of meals and housekeeping chores such as bed making, dusting and vacuuming.

Qualifications include a mature person who is at least 18 years old. High school diploma or GED preferred. Is able to understand and carry out directions and instructions, record messages and keep records. Is able to read, write and speak English. Is physically and emotionally capable of performing the job requirements.

No experience necessary; we will train. Work in both Facility and Community settings.

 

Licensed Nursing Assistant – Pediatric

The LNA works for both community and/or facility settings. PREVIOUS LNA EXPERIENCE (MINIMUM OF 1-YEAR) AND NH L.N.A. LICENSE IS REQUIRED.

Is able to work independently, understand and carry out directions and instructions, record messages and keep records. Is able to read, write and speak English. Homemakers - Personal Home Services

Paraprofessional - Per Diem - Non-Benefitted

Performs homemaking tasks according to the care plan and the client's wishes. Thorougly complete all tasks such as dusting, damp/dry mopping, vacuuming, cleaning kitchen appliances and cleaning bathroom fixtures.

CRVNA offers on-the-job training to qualified candidates. No previous healthcare experience is required. Participants must demonstrate competency in several areas at the end of the training. Training begins with orientation on Monday (8am - 4pm) then continues with a variety of classroom and/or hands on experiences based on the staff members’ availability.

What kind of work is currently available for a Homemaker?

CRVNA currently has Homemaking work available in the home setting on a Per Diem basis. While this type of work doesn’t provide a guaranteed schedule of hours or a fixed schedule, it does offer a flexible schedule based on the staff member’s availability and the agency’s needs.

If you enjoy working independently, Per Diem work in our patient’s homes is available in Concord and the surrounding communities. Travel in all kinds of weather is required for this work.

Physical Therapist - Home Care

We are looking for an experienced therapist to join our team. If you are looking to make a difference in your patient's quality of life, we are looking for you! PT provides patient evaluations and treatment to patients in the home setting according to established policies and practice standards.
Bachelors or Master's degree in Physical Therapy from a program approved by the APTA and current licensure in NH. 2 years of experience in an inpatient and/or community health care setting.
