Physical Therapy - BAYLOR POSITION

Professional - Full Time - Benefitted

BAYLOR POSITION - We are looking for experienced therapists to join our team. If you are looking to make a difference in your patient's quality of life, we are looking for you! PT provides patient evaluations and treatment to patients in the home setting according to established policies and practice standards.

Bachelors or Master's degree in Physical Therapy from a program approved by the APTA and current licensure in NH. 2 years of experience in an inpatient and/or community health care setting.

Baylor position requires working 30 hours/week (3 days) and receive full-time pay and benefits. Work Tuesday, Wednesday and Friday.

 

Homemaker-Personal Home Services

Paraprofessional - Per Diem - Non-Benefitted

Performs light homemaking tasks for client's in their home such as dusting, damp/dry mopping, vacuuming, cleaning kitchen appliances, cleaning bathroom fixtures, laundry and making beds.

CRVNA offers on-the-job training to qualified candidates. No previous healthcare experience is required.
Participants must demonstrate competency in several areas at the end of the training. Training begins with orientation on Monday and Tuesday (8a-4p) then continues with a variety of classroom and/or hands on experiences based on the staff members’ availability.

What kind of work is currently available for a Homemaker?

CRVNA curreintly has Homemaking work available in the home setting on a Per Diem basis. While this type of work doesn’t provide a guaranteed schedule of hours or a fixed schedule, it does offer a flexible schedule based on the staff member’s availability and the agency’s needs.

If you enjoy working independently, Per Diem work in our patient’s homes is available in Concord and the surrounding communities. Travel in all kinds of weather is required for this work.

What are the requirements for a Homemaker?
·Must have a high school diploma or GED
·Must be at least 18 years of age
·Caring and compassionate
·Dependable and reliable
·Able to meet the training requirements
·Dependable Automobile with Insurance ($100,000/$300,000) for community work
·Acceptable Criminal Record Check report
·Completion of a Post Offer Pre-employment Physical

Must be able to understand and carry out directions and instructions, record messages and keep records. Is able to read, write and speak English. Is physically and emotionally capable of performing the job requirements.

 

Physical Therapy Assistant

Responsibilities include patient evaluations, implementation of the plan of care and treatment to patients in their homes in order to attain clinical and functional goals.

Candidates must be licensed as a Physical Therapy Assistant by state of New Hampshire. Graduate of a two year program approved by the American Physical Therapy Association. Minimum of one year Physical Therapy Assistant experience required.

Must be able to understand and carry out directions and instructions, record messages and keep records. Is able to read, write and speak English. Is physically and emotionally capable of performing the job requirements.

 

Homecare RN

The Concord Regional Visiting Nurse Association is seeking a Registered Nurse to deliver skilled nursing care to clients and families according to nursing standards. Responsibilities include implementing a plan of care and managing a caseload of patients. Travel is required for this position. Qualified RNs must have current NH nursing license; and a minimum of 2 years experience including 1 year of recent medical/surgical or community nursing.

A LNA performs personal care and designated health-related services for patients in their homes with end of life care needs. The care is provided to clients under the supervision of a registered nurse. A minimum of 1-year experience and NH L.N.A. license required.

Must be able to understand and carry out directions and instructions, record messages and keep records. Is able to read, write and speak English. Is physically and emotionally capable of performing the job requirements.

 

Home Health Aide- Home Care & Personal Home Services

Paraprofessional - Per Diem - Non-Benefitted

A Home Health Aide performs personal care and designated health-related services for patients in the home. The care is under the supervision of a Registered Nurse and can include nail care, foot care, bathing, dressing, transferring patients, housekeeping, meal preparation, etc. Positions require: NH LNA license; 1 year of LNA experience; auto insurance and travel.

Must be able to understand and carry out directions and instructions, record messages and keep records. Is able to read, write and speak English. Is physically and emotionally capable of performing the job requirements.

Personal Care Service Provider

What is a Personal Care Provider?

A Personal Care Provider or PCSP is a caregiver for patients who need assistance with self-care activities. Self-care activities may include:
·bathing or showering
·dressing (putting clothes on or off)
·grooming (shampooing hair, brushing teeth
·performing light homemaking tasks (sweeping, vacuuming, washing, drying and folding clothes, etc.)
·preparing meals
·reminding patients to take their medications
·assisting patients with getting up from sitting position, repositioning in bed, guiding patient using a walker, cane, etc.
·assisting patient with a home exercise program or other activities as determined by the patient’s care plan

What kind of training is required to become a Personal Care Provider (PCSP)?

CRVNA offers on-the-job training to qualified candidates. No previous healthcare experience is required. The training is approximately two (2) weeks and participants must demonstrate competency in several areas at the end of the training period before they can work as a PCSP. Training begins with orientation on Monday and Tuesday (8a-4p) then continues with a variety of classroom and hands on experiences based on the staff members’ availability.

What kind of work is currently available for a Personal Care Provider (PCSP)?

CRVNA has PCSP work available in both the facility and home settings.

Per Diem work is available for days, evenings/nights & weekends. While this type of work doesn’t provide a guaranteed schedule of hours or a fixed schedule, it does offer a flexible schedule based on the staff member’s availability and the agency’s needs.
