Clincial Documentation Review Nurse
Administrative - Part Time - Benefitted
Responsibilities include review of clinical documentation to ensure that CRVNA’s clinical Home Care documentation is complete and meets acceptable standards. Clinical documentation review includes all Medicare admission OASIS, Plan of Care, Resumption of Care and Re-certification OASIS documents and Plans of Care. The qualified candidate will be an RN (BSN preferred); current NH nursing license; knowledge of Medicare COP, Clinical Standards of Care and documentation requirements and recent home health experience preferred.

Responsibilities include implementing a plan of care and managing a caseload of patients. Travel, weekend and holiday rotation is required for this position. The qualified candidate will have a RN diploma and Associate Degree (BSN preferred); current NH nursing license; recent acute care experience skills and a minimum of two years nursing experience.

SMS Care Coordination Program Coordinator

This position is responsible for Care Coordination for children in the State of New Hampshire’s Special Medical Services (SMS) program. Responsibilities include evaluating, planning, and coordinating care needs and services for an assigned medical specialty population of children, provides consultation and education to caregivers and families in the community within Special Medical Services. This position also makes home visits as required. The qualified candidate must be a Pediatric Registered Nurse (BSN or MSN preferred) or a medical Social Worker MSW with 3-years of recent experience in Community Child Health Nursing. Equivalent professional background will be considered based on qualifications and team composition. Work Schedule: 2 days per week through 5/31/2010; 3 days per week thereafter. After this position changes to 3 days per week, it will be benefitted.

Homemaker-Personal Home Services

Performs homemaking tasks according to the care plan and the client's wishes. Thorougly complete all tasks such as dusting, damp/dry mopping, vacuuming, cleaning kitchen appliances and cleaning bathroom fixtures.
CRVNA offers on-the-job training to qualified candidates. No previous healthcare experience is required. Participants must demonstrate competency in several areas at the end of the training. Training begins with orientation on Monday and Tuesday (8am - 4pm) then continues with a variety of classroom and/or hands on experiences based on the staff members’ availability.
What kind of work is currently available for a Homemaker?
CRVNA currently has Homemaking work available in the home setting on a Per Diem basis. While this type of work doesn’t provide a guaranteed schedule of hours or a fixed schedule, it does offer a flexible schedule based on the staff member’s availability and the agency’s needs.
If you enjoy working independently, Per Diem work in our patient’s homes is available in Concord and the surrounding communities. Travel in all kinds of weather is required for this work.
